masters: regenerate (Opus 4.8) — fixed module titles + L4-5 review

Batch pass with the updated guide. Pushing the lessons that came out clean at 100% cassette coverage: book-1-lesson-4, book-4-lesson-1/2/4, and the new book-4-lesson-5 (ST-only review). All now use the correct "Book N · Lesson N: <title>" module-title form. Held for review (over-dropped cassette content this run, flagged by the coverage check): book-1-lesson-1/2/3 and book-4-lesson-3. book-1-lesson-5 (review) is blocked on 29 ungenerated ST images.
